Position Summary
The RN Bridge Nurse plays a vital role in supporting clinical operations by bridging staffing gaps in unassigned homes and working directly with patients and families to ensure acceptance and utilization of authorized care. This role ensures continuity of care, promotes patient engagement, and supports compliance with clinical and regulatory standards. When not actively filling staffing gaps, the RN Bridge Nurse contributes to clinical oversight through supervisory visits, documentation review, and nurse education.
Key Responsibilities
Utilization of Unstaffed Homes
- Provide direct nursing care in homes without assigned staff to maintain continuity and quality of care.
- Complete assessments and documentation (e.g., T1001) for patients in unstaffed homes.
- Identify and report trends in staffing gaps and collaborate with the DON and Care Team Directors to address them.
- Support interim care planning and execution until permanent staffing is secured.
Patient Engagement & Care Acceptance
- Engage patients and families to encourage acceptance of approved care services.
- Educate on the importance and benefits of home health services.
- Participate in care plan discussions and family meetings to resolve concerns or hesitations.
- Collaborate with Patient Coordinators and Directors to align care delivery with patient needs.
Clinical Oversight & Support
- Conduct supervisory visits for patients without assigned nurses or those assigned to LPNs.
- Review and sign Case Management notes to ensure compliance and quality.
- Support nurse education and mentorship to improve documentation, clinical judgment, and utilization practices.
- Notify physicians and interdisciplinary teams of changes in patient condition.
- Serve as a clinical resource for field staff and leadership.
Qualifications
- Active Registered Nurse (RN) license in the state of practice (BSN preferred).
- Minimum of 3 years of nursing experience; home health or transitional care experience preferred.
- Strong communication and interpersonal skills, with a focus on patient-centered care.
- Knowledge of DOL/EEOICPA guidelines and home health regulations a plus.
- Proficient in EMR systems; experience with IGEA preferred.

Work Environment
- Field-based with travel to patient homes as needed.

Responsibilities:
β’ Execute physician prescribed plans of care.
β’ Complete compliant documentation of the care provided in the system of record.
β’ Manage and administer prescribed medication, treatment, and therapies.
β’ Conduct patient assessments, coordination of care.
β’ Perform various activities associated with daily living.
β’ Monitor vitals and GI intake and output.
β’ Assess skin integrity and administer wound care.
β’ Monitor for changes in the patientβs condition such as weight loss/gain, self-care abilities, and indicators of disease progression.
β’ Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients.
β’ Provide education and training to the patient, their family, and/or the Home Health Aide.
β’ Work with patients to be done one-on-one in the patients home to provide for a comfortable environment.
β’ Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants.
β’ Perform other personal care services as necessary to meet the patientβs needs.
Minimum Qualifications:
β’ Education, licensing, and certification: graduate of an accredited college or university, Associate or Baccalaureate School of Nursing
β’ Current State or Compact License as a Registered Nurse or Licensed Practical Nurse.
β’ Current CPR certification
β’ TB test and physical examination.
β’ Maintain required insurance.
β’ Good interpersonal skills.
β’ Hand-eye coordination and manual dexterity.
β’ Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients.
β’ Ability to operate under stressful conditions and make quick decisions.
β’ Must have dependable transportation and be willing to commute to the patientβs home.
β’ Ability to provide basic medical management up to medically fragile end of life care.
